You are requested to go through the following instructions:

You should obtain a demand draft (DD) for Rs. 1000/- towards application fee, in favour of Director,SGPGIMS, Lucknow Academic Account, drawn at SBI,SGPGI Branch (Code No- 7789) payable at Lucknow.

You should have an E: MAIL id . Every correspondences will be done through E: MAIL.

Keep with you the details/ particulars/ copy of 10th Std / MBBS / MD/MS qualification certificates/ mark sheets , Medical Council Registration Number, Caste Certificate(If applicable) etc before starting to fill in the online application.

Now fill up the application form for the appropriate program including the details of DD and submit online. 

You must take a print out of the completed application In 3 Pages (2 page application form and 1 page Identification cum Attendance card) after successful submission.

Now arrange documents in Following order

1. DD of Rs 1000/(One Thousand only) in favour of Director, SGPGIMS, Lucknow Academic Account, drawn at SBI,SGPGI Branch (Code No- 7789) payable at Lucknow. You MUST write REGISTRATION NO. printed at RIGHT corner of each page of application form and name of the candidate on the back of  DD.
2. Print out of the online application form In 3 Pages (2 page form and 1 page Identification cum Attendance card) with your signature at two places (page 2 of 3  and on Identification cum Attendance card) and paste passport size recent coloured photograph at the appropriate places.
3. Self certified copy of Matriculation certificate or any authentic age proof certificate.
4. Self certified copy of MD/MS or equivalent degree, marks sheet/certificate.
5. Self certified copy of Caste certificate (If applicable) from competent authority.
6. Sponsorship certificate from the employer (if applicable).
7. If employed, candidate must ensure that employer signature is at Page 2 of 3 of the application form.

Duly completed application form should be sent in a envelope superscribed “Application for SR(HS)/PDF, January 2009 session and Registration number written on right side of the application form to
